KARATE WEST WAIVER & AGREEMENT

This Waiver & Agreement (“Agreement”) is entered into between Karate West (“School”) and the undersigned participant or parent/guardian (“Participant”). By signing below, Participant acknowledges and agrees to all terms in the three sections that follow.
1. Health & Liability Waiver
I, the undersigned participant or parent/guardian of the minor participant (“Participant”), understand that martial arts training involves strenuous physical activity and carries inherent risks of injury or illness. I certify that Participant is in good health and able to safely engage in these activities.
In consideration of being permitted to participate, I assume all risks, known and unknown, and release and hold harmless Karate West, its owners, instructors, employees, agents, and volunteers (“Released Parties”) from any and all claims, demands, or causes of action for injury, illness, property damage, or death arising out of or related to participation, whether caused by the negligence of the Released Parties or otherwise.
I further agree to indemnify and defend the Released Parties against any loss or expense, including reasonable attorney’s fees, resulting from claims related to Participant’s involvement in Karate West activities.
2. Photo / Video Release
I grant Karate West permission to photograph and/or record video of me or my child during classes, events, or demonstrations. These images may be used for marketing, social media, website content, or other lawful promotional purposes without compensation. I waive any right to inspect or approve the finished product or any text that may accompany such images.
3. Payment Authorization
I authorize Karate West to process tuition and related program fees using the payment method I provide (credit card, bank draft, or other agreed form). Charges will occur according to the current pricing and billing policies communicated at enrollment. I understand that I may update or cancel my payment by submitting a cancellation request through my Gymdesk account. This serves as the required 30-day notice, and tuition stops once that notice period is complete. I remain responsible for any balances owed through the effective cancellation date.
By signing below, I confirm that I have read, understood, and voluntarily agree to all terms of this Agreement.
